To Place A Service Call:Please complete the Service Request Form and do one of the following: Print and fill out the Service Request Form, and fax it in to the CWS Service Department at (352)368-4775 Fill out the Service Request Form and Submit it directly to the CWS Service Department. (The submit button is located in the upper right hand corner of the PDF) Please see below if you plan to submit the form directly.
ATTENTION: If you choose to submit your form directly to our Service Department, please be sure to save or print a copy; to save a copy, just click on the save button located in the upper left hand corner of the PDF file. Next, click on the submit button (the submit button is located in the top center on the Service Form); please choose the proper emailing option. If you do not choose the proper email option, then your form will not go through. Also, if you have not been contacted within 24 hours, please call our service department at (800)550-1081.
